Good Conduct Certificate requirements (application through the Qatar Embassy)

To apply for Good Conduct Certificate from the Embassy of Qatar in Ottawa you are required to provide a number of supporting documents:

• A cover letter stating your request and the purpose.
• A copy of the passport used in Qatar.
• A copy of your old or expired residence visa in Qatar.
• A copy of your current passport.
• A copy of your current valid residence permit/citizenship in Canada (PRC, Work Permit, etc.).
• A fingerprint form (C-216C) which may be obtained at a police station (notarized, authenticated and legalized) – if you had a Qatar Residency Permit after the year 2012, you are exempt from submitting a Canadian Fingerprint form.
• Four passport size photographs (size: 4.8cm x 3.8cm).
